R-7 High Schools to Offer 2010-11 Sports Physicals

PLEASE NOTE BOTH PHYSICAL DATES WILL BE HELD AT LEE’S SUMMIT WEST HIGH SCHOOL

According to the Missouri State High School Activities Association, students participating in high school athletics, dance team, color guard (flags) and cheerleading must have on file at the school a physician’s certificate stating that he/she is physically able to participate in practice and competitions.  These forms are available in the Activities Office.  To participate in or tryout for any of these activities for 2010-11 school year, the student must have a physical on file that is dated on or after February 1, 2010. The physicals dated on or after February 1, 2010 are good through the last day of school 2011.  Summer practices and sport camps will require a new physical dated on or after February 1, 2010.

Athletic coaches and sponsors will not accept physicals.  Students and/or parents turning in a physical MUST turn them in to the Activities Office.  Before participating in 2010 spring sports, students should have a current physical on file in the Activities Office.  Students may check a list in the Activities Office to see if they have a current physical.

**Lee’s Summit West High School will be offering physicals on Saturday, February 20, and on Saturday, February 27. These physicals are only for current students in the R-7 School District who will be in grades 9 through 12 for the 2010-11 academic school year. Physicals will begin at 8:00 AM and doors will be closed at 10:30 AM.  The cost is $20.00.

Pre-registration is required for both physical clinics.

Pre-registration can be done through the LSHS (986-2107), LSN (986-3808), and LSW

(986-4008), Activities Office.  Those not pre-registered will be turned away at the door.

A valid physical must have the following information on the form:

To be completed by parents:

  1. Verification that he/she has basic athletic insurance coverage including company name and member identification insurance number.
  2. Demographic and emergency information.
  3. A statement signed by the student’s parents or guardians granting permission for the student to participate.
  4. Student’s signature.
  5. The Medical History section on side 2 of the form completed prior to the physician’s physical.
  6. Correct date making the physical valid for the appropriate school year.

To be completed by doctor:

    1. Medical information on side 2 of the form-indicating student is physically able to participate.
    2. Physician’s signature and the date signed by the physician.

The coaches and staff at LSW hope this information will be beneficial to parents in preparing their son or daughter to participate in high school athletics and activities within a timely manner.  The school-offered physical dates are a great opportunity for student athletes to take care of this important obligation so they may continuously participate in sports and activities here at LSW.
